Exhibit A — The determination. One page, fourteen numbered fields, twelve of them filled in by somebody who has done this before. The only box larger than the age is the box that would say why the age did not matter, and that box has been covered with a bar measuring 4.1 by 2.7 inches. The form provides a continuation sheet in case the criteria run long.
